Has there been any tinkering with the refine system?
I've tried upgrading armor from +4 to +5 and all 10 failed.
According to RMS +4 to 5 has a succes chance of 60%, so yeah, I just wanted to ask to make sure.

Sounds like it is being unlucky.
I have gotten things to +6 before it broke.
