In order to create an embryo, you need:

1x Normal Potion Create Book / Potion Creation Guide       
1x Medicine Bowl
1x Seed of Life       
1x Life Force Pot / Glass Tube
1x Yggdrasilberry Dew / Morning Dew of Yggdrasil       

In addition, you'll need the alchemist skill Pharmacy to create it. All of these ingredients are sold by the Alchemist NPC, who's northeast of Prontera's central fountain.

I find that if you're low lvl buying Embryo for 100 to 130 k each is easier (and sometimes cheaper) than failing potion creation
