Works anywhere as far as I know.

Limitations:
Account-bound.
Can't be traded, cant be dropped, cant be mailed.

I remember in an old event where it was non-SQI and non-MVP only, so the Kafra Clip wasn't allowed on the character upon entering the map.

Not sure if this is the case in similar scenarios.

It's an accessory, so you can use it whenever you want.

I doesn't work when you're dead. That might seem obvious till you try to open kafra to get an ygg leaf.

you can't switch gears when you're dead... so it's obvious that you can't equip/unequip kafra clip while being dead....

I'm just pointing out some obvious logical explanations of why kafra clip doesn't work when you're dead.
